Costs, term and notice periods of an entry

The entry on prophylaxe-berater.de is subject to a charge. To cover our costs we levy an annual fee of €58 including statutory VAT. The entry runs indefinitely but may be cancelled at any time with one month's notice; any pro-rata amounts of the annual fee are not refunded.

Your entry appears in the practice search of prophylaxe-berater.de.

Data collected in the prophylaxe-berater.de database when an entry is made

When you complete the online form, the data recorded there is used for internal administrative purposes. In addition, the data you provide in the fields of the registration form (with the exception of the password) is published in the practice search of prophylaxe-berater.de.

Consent

In order to publish your data in the practice finder, we depend on your consent. These data will not be used beyond the purpose described here. You may withdraw your consent at any time with effect for the future.

The wording of the consent you gave with your declaration of participation: "I agree that the content of the fields name, address, town, postcode, telephone, fax, e-mail, URL and areas of focus will be published in the practice search of prophylaxe-berater.de."

At the end of the registration process you will find a button labelled "Enter details in database". By clicking that button you declare that you have given this consent.
